Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.
Summary:
The Social Worker will be responsible for the support/intervention of patients and families with psychosocial needs and coordinates the development of a discharge plan of care for complex patient populations.
The position will assist patients and families in adjusting to episodes of acute and chronic illness; cope with lifestyle changes precipitated by illness or disability; participate in restorative health programs; carry out discharge recommendations; and in addressing issues of end of life care.
Using a variety of treatment modalities, the Social Worker will assist patients and families in the alleviation or resolution of social, financial and emotional problems related to illness, health care, and rehabilitation.
This position is provides patient/family interventions to support clinical, satisfaction, financial, and compliance related outcomes.

Responsibilities:
1.
Provides psychosocial/resource management expertise to the Nurse Case Manager and care team on complex cases.
2.
Collaborates with interdisciplinary team, community agencies and the healthcare team for problem resolution with patient and family communication.
3.
Facilitates legal, advocacy and ethical issues related to domestic violence, abuse, and medical care planning.
4.
Assesses patients and family's psychosocial risk factors through evaluation of prior functioning levels, compliance related issues, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
5.
Performs psychosocial assessments, and intervenes with patients and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs6.
Conducts support groups, crisis management and interventions on matters related to adjustment to illness, treatment decisions, end of life issues and grief or loss.

Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed services with the health care system8.
Provides education and intervention in abuse/neglect/exploitation, guardianship foster care, adoption, mental health placement, advance directives, or other ethical issues9.
Participates in discharge planning activities for complex patients to ensure a timely discharge and to provide linkage with post-discharge care.

Education and Experience Requirements:
oMaster Social Worker (MSW) or equivalent experience.
oMinimum of one to three (1-3) years' experience in Social Work or similar field of practice.
oPREFER:
Case Management experience in an acute care setting.
Minimum Knowledge, Skills and Abilities Requirements:
oMust have strong interpersonal and communication skills.
oAnalytical abilities to assist in working independently, obtaining solutions to problems and prioritize work.
oMust be able to manage multiple competing priorities and maintain calm demeanor in stressful environment.
oProficiency with computers and clinical applications.
License, Registration, or Certification Requirements:
oCurrent NYS license in Social Work or eligible for limited permit.
If limited permit, must obtain license within 12 months of hire date or employment will be terminated.
oPREFER:
Licensed Clinical Social Worker (LCSW).
oPREFER:
Certified Case Manager (CCM).
