As a member of the Hudson Valley Hospice Interdisciplinary Team, the primary focus of the Home Care Social Worker is to support patients/families/staff through end of life issues, decision making and anticipatory grief.
The social worker provides emotional support/counseling in addition to concrete social services as indicated and actively participates in developing the patient/family care plan.
The primary focus of the Bereavement Social Worker is too provide grief support to families and loved who have lost their loved on hospice.
We are looking for full-time Social Workers for our Home Care teams, both Ulster and Dutchess Counties and our Bereavement Department.
Per Diem work needed as well! Social Worker, Home Care:
Ulster and Dutchess territories available! 40hrs per week Monday-Friday (8:
30am-4:
30pm).
No Weekends or Mandating! Travel Required.
Bereavement Social Worker:
Poughkeepsie Office and Kingston Office locations, 40hrs per week, Monday-Friday (8:
30am-4:
30) with one late evening.
Education and Experience we are looking for:
Master's degree in social work and valid New York State Social Work License or Limited Permit eligible--LCSW preferred.
Minimum of two years of social work experience with one-year experience in medical social work, grief counseling, hospice and/or community health preferred.
Knowledge of family and group treatment:
unique issues and dynamics related to death, dying and grief preferred.
Excellent counseling, advocacy and collaboration skills.
Valid New York State driver's license with reliable transportation.
