The Social Worker Assistant 3 (SWA3) will provide oversight to the Family Care Program in Dutchess, Putnam and Ulster, counties, while covering a caseload of 10-25 Family Care Homes. The SWA3 functions as a Family Care Home Liaison (FCHL). The SWA3 will ensure the Family Care Providers are utilizing consumer funds per DDSO guidelines. o Responsible for ensuring that the safety and health of the individuals are maintained in the Family Care Home. o Responsible for monitoring the homes to ensure that any safety and health issues are immediately address and that the required follow up is completed o Assist with emergency movements and placements. o Ensure communication occurs between the Family Care Providers and the other members of the treatment team o Monitors and develops Staff Action plans and Safeguard Summary plans and ensure that they are consistent with the individuals' Life plan. o Monitor Family Care Homes to ensure living environments meets the individuals physical, medical, social, and spiritual needs. o Acts as a liaison between the Family Care Provider, the individual and the other treatment team members. o Ensure that the Family Care Providers adhere to family care policies relative to health and safety of the individuals in their Family Care Home. o Ensures that the FC policy 1.5 (Family Care Home Liaison Responsibilities) are adhered to. o Ensure that the Family Care Providers are following regulatory requirements. o Ensure that individuals receive their benefits as delineated by regulations. o Ensure that Family Care Providers are adhering to the individual plans of care. o Other duties as assigned.

Salary Range:
From $59497 to $75326 Annually

Qualifications:
Candidates must currently be a permanent Social Work Assistant 3 OR be reachable on the current Civil Service eligible list Transfer
Qualifications:
Candidates must be eligible for transfer under Section 70.1 of the Civil Service Law. Provisional
Qualifications:
Candidates must have a master's degree in social work OR a bachelor's degree in social work and ONE year of post-bachelor's experience in social work OR a bachelor's degree in a human services area and TWO years post-bachelors experience in Social Work. OPERATIONAL PREFERENCE:
QIDP (Qualified Intellectual Disorder Professional) designation requires one year's experience working with people with a developmental disability AND a bachelor's degree or higher in a human services field (including, but not limited to:
sociology, special education, rehabilitation counseling or psychology).Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
